Who are we?
Sole Bay Care Fund, is a local community charity. It works with other community organisations and groups to support the local community within the Sole Bay Area. Our 'community is one that cares' and works together to try and ensure health and social care needs are met. The fund is predominantly for those patients registered with Sole Bay Health Centre, Reydon, Nr. Southwold, but it also supports wider community projects in order to support the welfare, health and social engagement of the community as a whole.
Our main aim is to help manage the Health and Social care needs not normally funded by the NHS or Social Services. We aim to keep people at home or near to home for as long as possible, by providing the support and equipment they may need if unwell or isolated or needing additional short term support. We specialise in supporting older people, palliative care and those living with dementia, or help with specialist needs. Please contact us via, Sole Bay Health Centre if you need further information.
